About Us
Heart to Home, Inc. was founded in 2007 as an alternative to traditional large-scale assisted living, memory care and nursing home facilities. With the realization that most long term care was being delivered in clinical, impersonal, fast paced settings we wanted to create something different. Our unique homes house a small number of residents (6-7) and are organized in a way intended to maximize normal living environments and routines, resident autonomy, sense of community, and quality of life. In addition our staff take a holistic approach to care focusing on the well-being of the whole person. Mind. Body. Soul.

Paulette Vrem, Director of Nursing - (612) 245-4987
Masters Degree in Nursing (MSN), Masters Degree in Community Health, Board Certified Gerontological Registered Nurse (RN), Co-Founder
Paulette has worked extensively as a geriatric nurse, homecare nurse and patient advocate since 1982. She is Heart to Home's onsite managing RN and care coordinator. She works directly with our visiting doctor, Dr. Todd Stivland (Bluestone Physician Services) to ensure your loved one receives the best possible care. Paulette has a great sense of joy and passion enhancing the lives of our residents. Her career has been dedicated to improving the lives of seniors and improving long term care.

Certified Housing Manager, Certified Nursing Assistant, Serve Safe Certified, Co-Founder
Amy brings a breadth of knowledge and passion to Heart to Home. She has been a direct caregiver of aging adults for over 25 years. She actively manages Heart to Home's residential care homes, provides on-going mentoring to our Certified Nursing Assistants and is often referred to as the "Martha Stewart" of our homes.

Josh is passionate about helping our residents and their families. He enjoys working with the residents, their families and managing Heart to Home's business operations. He is also a current board member with the Saint Paul Senior Workers Association and Residential Care Providers Network.

Nick, a residential builder for over 35 years, brings an immense amount of building design and construction knowledge to Heart to Home. To provide safe and beautiful accommodations for our residents is a monumental priority.
